Quality Improvement Projects - QI Science Methodology
Teams test tool(s) using quality improvement science. These projects follow the American Board of Pediatrics Maintenance of Certification (MOC) Standards and have potential to award pediatricians credit toward MOC Part 4 when they participate in these projects.

Bright Futures Preventative Services Improvement Project (PSIP)
The aim of this project is to answer “Can Bright Futures be easily implemented, birth to 3, in a busy clinical setting?” by testing tools, strategies and measures designed to improve Bright Futures recommended preventive services for children age birth to 3. These tools, strategies, and measures will be based on the Bright Futures Guidelines 3rd edition and the Bright Futures Tool and Resource Kit. Up to 20 QuIIN members and their staff will be recruited to participate in this 15 month project.

Newborn Screen Positive Infant ACTion Project
**Approved by the American Board of Pediatrics for MOC Part 4 credit!
The Newborn Screen Positive Infant ACTion Project aims to improve appropriate responses in the short-term management of infants affected by congenital conditions, identified through newborn screening and to assess the effectiveness of systems of care, including use of the ACT sheets, in assisting the pediatrician to provide appropriate responses in the short-term management of infants.

Safe and Healthy Beginnings Improvement Project
**Approved by the American Board of Pediatrics for MOC Part 4 credit!
The Safe and Healthy Beginnings Improvement Project, the pilot project of the AAP Quality Improvement Innovation Network, was designed to ensure a safe and healthy beginning for all newborns by testing measures, strategies, and tools based on aspects of the AAP's revised hyperbilirubinemia guidelines.

Safe and Healthy Beginnings Toolkit Evaluation Project
The Safe and Healthy Beginnings Toolkit is the result of the quality improvement project through the QuIIN with the purpose of testing these tools for implementation of the AAP's evidence based clinical practice guideline, "Management of the Hyperbilirubinemia in the Newborn Infant 35 or More Weeks of Gestation". The goal of the SHB Evaluation Project is to strengthen the toolkit and identify possible strategies to promote the toolkit and facilitate its implementation.

National Performance Measures: Care for Children with Otitis Media with Effusion
**Approved by the American Board of Pediatrics for MOC Part 4 credit!
This project tested the feasibility and reliability of a set of performance measures to assess the care of children with otitis media with effusion (OME). These measures developed by the AMA-convened Physician Consortium for Performance Improvement and the American Academy of Otolaryngology-Head and Neck Surgery and selected by the Ambulatory Care Quality Alliance for use by insurers and health plans have not been evaluated in practice. The measures were then linked with continuing education and quality improvement methods to test strategies that allow pediatricians to improve the effective use of therapeutics in children.

Practicing Safety: A Child Abuse and Neglect Prevention Project
**Approved by the American Board of Pediatrics for MOC Part 4 credit!
The aim of this project was to decrease child abuse and neglect by expanding anticipatory guidance and increasing screening for possible child abuse and neglect by pediatric practices to parents of children ages 0-3. QuIIN members implemented tool bundles (Parent Tools, Child Tools, Infant Tools) to determine feasibility and efficiency of bundled tools. For additional information click on the link above.

Additional Improvement Efforts
QuIIN members provide practicing pediatrician input into feasibility of innovations (guidelines, measures, tools) as well as identify tools to support implementation where appropriate. Additional improvement efforts are categorized below as Innovations/New Products, EQIPP Testing, and Evidence-Based Clinical Practice Guidelines.

Innovations/New Products

Adoption and Meaningful Use of Electronic Health Records (EHRs) Project
The AAP Child Health Informatics Center (CHIC) is seeking input into the usefulness of their current resources, as well as input into the development of additional tools and resources, to assist the pediatrician in meeting the criteria to qualify as Meaningful Users (MU) of EHRs. QuIIN members will be invited to participate in an educational conference call to learn about MU prior to completing a readiness survey to 1) assess readiness to participate in MU and 2) to identify tools needed to assist the pediatrician in participating in MU. Once survey results are tallied, 8-10 QuIIN members will be recruited to implement the MU requirements in practice and participate on calls   to identify gaps and tools to assist pediatricians seeking to be meaningful users.

2010 Guidelines and Tools to Improve Care for Children with ADHD Improvement Project
This project, conducted in partnership with the National Initiative for Children's Healthcare Quality, will pilot components of the “Caring for Children with ADHD: A Resource Toolkit for Clinicians” based on the newly revised AAP ADHD guidelines which includes an expanded scope of ages 4-18.  Based on the experience and feedback provided after using these tools with patients, these tools will be revised and finalized prior to national distribution. This project is made possible by funding provided by McNeil Pediatrics, Division of Ortho-McNeil-Janssen Pharmaceuticals, Inc. which supports the partnership between QuIIN and NICHQ to facilitate the update of the ADHD Toolkit. BlueCross BlueShield Association Pediatric Obesity and Diabetes Prevention Physician Field Testing
The AAP and the BlueCross BlueShield Association (BCBSA) have been working together over the past several months to develop a physician tool kit pilot targeting diabetes prevention and pediatric obesity.  The pilot BCBSA tool kit was created to educate physicians, parents, and children on diabetes prevention and strategies to live healthier lives.

In an effort to continue this collaboration, BCBSA invited QuIIN Members to participate in the field testing of this physician tool kit. Additional information can be found at the link above.

National Committee for Quality Assurance (NCQA) Child Health Project
The aim of this project was for the AAP to provide practicing physician input into the draft NCQA Child Health Project performance measures and for QuIIN members to participate in field testing these measures prior to dissemination.

Newborn Screen Positive Infant ACTion Project - Phase 1, Utility Project
Phase 1 consisted of a test of utility of 10 of the ACMG’s ACT sheets. Participating physicians reviewed 10 ACT sheets and completed a brief on-line survey (via Survey Monkey) providing feedback about the ACT sheets. Questions focused on the sheets’ utility of the information, format, and level of detail provided in the forms.

UNC/AAP ADHD Medicine Abuse: What Parents Need to Know
The aim of this project was to educate parents on the misuse of ADHD medications by developing and testing a parent brochure. Physicians completed a pre/post electronic survey to evaluate a new parent brochure and distributed these brochures to the parents of their adolescent patients who in turn, visited a Web site to evaluate the brochure. Test for Respiratory and Asthma Control in Kids
The aim of this project was to collect data and provide qualitative feedback on the new paper based tool being developed by Astra Zeneca to improve the identification of pre-school age children at risk for respiratory or control problems based on National Institutes of Health indicators.

Bright Futures Oral Health Risk Assessment Project
The Bright Futures Education Center and Oral Health Initiative partnered with QuIIN to assess the feasibility of the data collection tool that has been developed for a project looking at the viability of implementing oral health risk assessments into pediatric health supervision visits. Participating physicians used the Bright Futures Oral Health Assessment tool prospectively in 10 charts of children ages 9-12 months and provided feedback through an online survey and conference call. EQIPP Testing

EQIPP Bright Futures Course Testing
QuIIN member primary care practice physicians tested the Bright Futures Course. Participating physicians reviewed the course, including use of chart data and completed an on-line survey to help the EQIPP planning group make any final adjustments to the course before it is released.

EQIPP Data Collection Tool: Immunization Course
QuIIN member primary care practice physicians tested the feasibility of the data collection tool that has been developed for the upcoming Education in Quality Improvement for Pediatric Practice (EQIPP) Immunizations course. Participating physicians completed the data collection tool on 5 charts of children ages 19-23 months and 5 charts of adolescent patients ages 13-18 years. Participants also completed an on-line survey to help the EQIPP planning group make any final adjustments to the data collection tool prior to inclusion in the course.

EQIPP Diagnosing and Managing Asthma in Pediatrics Course Testing
QuIIN member primary care practice physicians tested the Diagnosing and Managing Asthma in Pediatrics Course, including the Quality Improvement Basics module. Participating physicians reviewed the course, including use of chart data and completed an on-line survey to help the EQIPP planning group make any final adjustments to the course before it is released.

EQIPP Data Collection Tool: Differentiate and Manage GER and GERD Course
QuIIN member primary care practice physicians tested the feasibility of the data collection tool that has been developed for the upcoming Education in Quality Improvement for Pediatric Practice (EQIPP) GER/GERD course. Participating physicians completed the data collection tool on 10 charts of children with the diagnosis of GER or GERD. Participants also completed an on-line survey to help the EQIPP planning group make any final adjustments to the data collection tool prior to inclusion in the course.

Evidence-Based Clinical Practice Guidelines

AAP Type II Diabetes Mellitus (DM) in Children and Adolescents Subcommittee
A QuIIN member primary care physician will represent QuIIN on the Type II DM in Children and Adolscents Clinical Practice Guideline Subcommittee and:
  • Provide a primary care perspective as these guidelines and recommendations are being created
  • Assist this group in identifying tools/strategies to implement the recommendations being created
  • Attend the March 28-29, 2009 guideline meeting and participate in emails throughout the guideline process

AAP Fever in Infants Under 3 Months Guideline Subcommittee
A QuIIN member primary care physician will represent QuIIN on the Fever in Infants Under 3 Month Clinical Practice Guideline Subcommittee and:
  • Provide a primary care perspective as these guidelines and recommendations are being created
  • Assist this group in identifying tools/strategies to implement the recommendations being created
  • Attend 2 in-person planning meetings, the first prior to June 30 and another 6 months later.
  • Participate in emails throughout the guideline process.

AAP Acute Otitis Media Subcommittee
A QuIIN member primary care physician will represent QuIIN on the Acute Otitis Media Subcommittee and:
  • Provide a primary care perspective as these guidelines and recommendations are being created
  • Assist this group in identifying tools/strategies to implement the recommendations being created
  • Attend subcommittee planning meetings
  • Participate in emails throughout the guideline process.

Completed projects are those in which participants have met the minimum criteria for participation. This typically occurs following data collection and a second learning session if applicable.

Projects in progress are those in which recruitment has closed, but the project is currently ongoing.

Upcoming projects include those that are still in the planning phase, but have been approved by the QuIIN Steering Committee as potential future projects.
